Every question × engine produces one of four verdicts. They are ordered by value.
Cited. The engine used your website as a source and linked it. The strongest result: you are not just mentioned, you are the evidence.
Recommended. You are put forward as an answer, usually with a reason. This is what most people mean by "does AI recommend me".
Named. You appear, but as one of a list, without a particular case being made for you.
Absent. You are not in the answer at all. The starting point for most businesses, and the thing the Fix Pack exists to change.

When a cell says failed
That means the engine errored on our side. Those cells are excluded from your score and the credit is refunded automatically. They are a fact about that engine, not about you.
